2011 Membership Application

Please fill out this form for our records. Upon completion of the form you will be given a username and password which will be displayed on the screen. You should copy these down in a safe place. As a backup, those credentials will be emailed to you at one or both of the email addresses you provide. We will send the credentials to the same email address that you choosed to receive the Cardinal Newsletter. You may choose both. If you do not choose to receive the newsletter at either email address then we will send the credentials to your home email address. The credentials will be required to login to the website which will get you access to the members only area. We plan on implementing PayPal soon, so once that is in place you may select to pay your dues by PayPal so that your membership will become active as soon as the PayPal transaction is completed.

When you have finished this filling out this form, you will be able to print a PDF version with the information filled in. If you do not wish to provide all of the data we ask for, then you can supply it on the printed form and mail it in at the address noted on the PDF.
